📡 TELEHEALTH & FINANCIAL POLICIES

⭐ TELEHEALTH CONSENT

By scheduling or participating in a telehealth visit with Jahan’s Wellness Lounge, you acknowledge and agree to the following:

1. Nature of Telehealth Services

Telehealth involves the use of electronic communications to conduct medical consultations, assessments, follow-up visits, and treatment planning. Telehealth may include:

  • Video calls

  • Telephone visits

  • Secure messaging or electronic forms

  • Reviewing labs, images, or medical records electronically

  • Telehealth is used when an in-person visit is not required or not possible.

2. Benefits of Telehealth

  • Increased access to care.

  • Reduced travel time and cost

  • Convenience of receiving care from home

  • Ability to review results and treatment plans electronically

 

3. Limitations & Risks of Telehealth

While telehealth is safe and effective for many conditions, it has limitations, including:

Inability to perform physical examinations

Technical issues (poor internet, dropped calls, audio/video failures)Potential, though rare, privacy/security risks with electronic communication

Possibility that telehealth may not be appropriate for certain medical concerns. If a condition requires an in-person visit, labs, or emergency evaluation, you will be directed accordingly.

 

4. Location Requirement

You must be physically located in California at the time of your telehealth visit, as our provider is licensed only in this state.You agree to truthfully confirm your location at the start of each telehealth session.

 

5. Privacy & HIPAA

ComplianceYour telehealth visit and medical information are protected under HIPAA. We use secure platforms for communication whenever possible.However, you are responsible for:Choosing a private location for your visit. Ensuring no unauthorized individuals can overhear or view your session.

6. Consent to Receive Telehealth Services.

By continuing with a telehealth appointment, you give your consent for: Evaluation and treatment via telehealth

Review of medical history, medications, labs, and records electronically

Transmission of health information electronically as needed

You may withdraw consent at any time by discontinuing telehealth services.

 

⭐ FINANCIAL POLICIES

These policies apply to all services, including weight loss programs, hormone therapy, vitamin injections, cash-pay visits, and Direct Primary Care (DPC) membership

1. Payment Responsibility

  • Jahan’s Wellness Lounge is a cash-pay clinic. We do not bill insurance for visits, consult fees, or membership fees.You are responsible for all charges at the time services are rendered, including: Consultation fees. Monthly membership fees.

  • Medication costs (if not included in program)

  • Lab fees (if applicable)

  • Program packages or injections

2. Direct Primary Care (DPC) Membership

Our DPC program is a monthly subscription for primary care access.By joining the DPC membership, you acknowledge: Membership fees are billed monthlyFees cover access to care, not insurance or per-visit billingMembership is not transferable or refundable once billed. A 30-day written notice is required to cancel the membership. DPC membership does NOT replace health insurance. Patients are encouraged to keep separate insurance for emergencies, specialists, imaging, and hospital services.

3. Weight Loss & Hormone Therapy Programs

Program fees may include consultations, follow-up visits, and provider support.

Medication costs (GLP-1, Contrave, phentermine, TRT, BHRT creams, etc.) may be billed separately by our clinic or by third-party pharmacies. You acknowledge: Medication availability and pricing may vary. Insurance rarely covers weight-loss medications. We cannot guarantee medication approval or pharmacy processing timesNo refunds are issued once medication is ordered or dispensed.

4. Refunds & Cancellations

To ensure fairness and operational efficiency:

  • Consultation fees are non-refundable, even if treatment is not pursued

  • Program or package fees are non-refundable once service begins

  • B12 or injection packages must be used within 12 months of purchase

  • Missed appointments or no-shows may result in a fee

  • Frequent cancellations may require prepaid appointments. 

If you need to cancel an appointment, please notify us at least 24 hours in advance.

 

5. Pharmacy & Medication Policies

Many medications (GLP-1, semaglutide/tirzepatide, TRT, BHRT, peptides, etc.) are filled by outside pharmacies.

You agree that:

  • Jahan’s Wellness Lounge is not responsible for delays due to pharmacy processing, shipping, or supply issues

  • Once a prescription is sent or a compound is ordered, we cannot reverse or refund it

  • Medications cannot be returned under pharmacy laws

  • Dose changes may require new prescriptions or charges

 

6. Labs & Diagnostic Testing

Lab work may be required before or during treatment.

  • Some labs may be ordered through your insurance

  • Others may be cash-based

  • You are responsible for lab charges directly to the lab facility

We will always discuss lab requirements beforehand.

7. Communication Policy

By providing your phone number and email, you consent to receive:

  • Appointment reminders

  • Follow-up instructions

  • Program updates

  • Wellness-related communication

You may opt out at any time for marketing messages, though clinical messages may still be required.

 

8. Emergency Care

Telehealth visits are not appropriate for emergencies.

If you have a life-threatening condition (e.g., chest pain, trouble breathing, severe bleeding), call 911 or go to the nearest emergency room.
